Drug Coverage - 2012 plan year information

Here you can find our lists of covered drugs (formularies) including Part B drugs and Enhanced drugs covered if you are in an Enhanced Part D plan with your former employer; descriptions of our pharmacy management programs; and lists of drugs that have Prior Authorization, Quantity Limit and STEP Therapy requirements.

You should review these documents to help you determine tier status of your medications, if your drugs cannot be filled through the mail order service, or if your medications have certain limitations.
