Health Care Management Programs
Living well can sometimes take an extra helping hand. That’s why we have a team of nurses, respiratory therapists, social workers and other health care professionals to help our members. These programs are available at no cost to MVP Health Care members.
Health Management
Health management programs offer help to members who need guidance and support to improve their chronic health conditions. These programs have innovative solutions to help get chronic health conditions on track and stay there.
- If the condition is in control, members can still benefit from health management programs. They will receive educational mailings that help keep them informed of important news about their condition, as well as help to remind them of available tests and treatments.
- At times, the condition may not be under control, risking complications or other health issues. When this happens, a personal Health Coach is available. The coach works with the member and his/her medical team to provide support and improve health. Together with the health coach, the member will identify health goals and work toward gaining control of the condition.

Disease Management
Disease Management programs helps those diagnosed with one or more health conditions to better manage their health by providing a focused effort on education about their condition. If the member has been struggling to stabilize his/her health, or was recently in the hospital, the program can help by:
- Finding and getting the medical services needed.
- Teaching members how to take the best care of themselves.
- Referring members to classes and services through the MVP Wellness Center.
- Locating other useful community resources.
- Working as a partner with the member and doctor.

Case Management
If the member has more pressing medical needs, or was recently in the hospital, the Case Management team is here to help. Case managers and social workers can help explore options available and navigate through a time of serious illness.
