The MVP Prescription Drug Formulary

MVP's Prescription Drug Formulary contains a list of covered drugs. Members may benefit from selection of a formulary preferred product through lower copays. Use the covered drug search to find out if a particular drug is covered on MVP's formulary and at what tier or click here for the 2011 formulary.

Some medications require prior authorization or are subject to quantity limits or step therapy as described in MVP policies. Prior authorization forms, new and updated versions, can be found on the Provider Forms page.

Electronic version of the formulary
MVP offers providers access to drug formulary information via ePocrates Rx ™, the leading handheld drug reference guide. MVP has contracted with ePocrates to allow providers to download the MVP formulary to a PDA device. In addition to the formulary information, you can compare copay tiers, check for multiple drug interactions, view medication dosing and precaution information.

Through the ePocrates program, the MVP formulary information is downloaded over the Internet onto a handheld PDA (Palm or Pocket PC) device compatible with the system. You will then be able to view this information when needed. If you do not yet use ePocrates, you will need to register before you can access the formulary.

If you are already an ePocrates user you can download the MVP formulary by going to the ePocrates Web site. The MVP formulary is available for free; you may also subscribe to other medical reference products offered by ePocrates.

Mail Order Program

MVP members who have prescription drug coverage can order maintenance drugs, those drugs taken on a regular basis through the Medco Pharmacy. Up to a 90-day supply of medication can be ordered through this program. Once a member's prescription is on file with the Medco Pharmacy, members can order refills by mail, phone or online and have their medication delivered to their door. The covered drug search can determine if a medication is available from the Medco Pharmacy. An MVP member who is taking a drug eligible for the mail order program may ask you write two prescriptions, one for up to a 30 day supply to be filled at a local pharmacy, and one for up to a 90 day supply to be filled through the Medco Pharmacy, MVP's Mail Order Pharmacy agent.
