Health Management Support

MVP DualAccess members work with a dedicated Care Team to address their physical, behavioral, and social health needs. Each Care Team includes a registered nurse care manager (RNCM), a community service care coordinator (CSCC), and a Licensed Clinical Social Worker (LCSW). Members can expect their Care Team to work with them on an ongoing basis to reach their individualized care plan goals. This happens through coaching, educational materials, connecting with clinical and community-based resources, and coordination with the member’s broader provider team.
