A Dual Special Needs Plan is a type of Medicare Advantage plan that focuses on intensive care coordination and is designed for individuals with special needs who uniquely qualify for Medicare and Medicaid. MVP’s D-SNP breaks down the traditional silos of care by empowering care managers to be in constant contact with members and their health care providers, ultimately allowing for a more coordinated approach to addressing a member’s physical, behavioral, and social health needs.
