Our Integrated Health initiative (PDF) is designed to enable behavioral health and primary care to succeed at integrating patient care, enabling them to support an individual’s journey to better health and optimal living.

STEP 1
All practitioners who require credentialing must complete the online Coalition for Affordable Quality Health Care (CAQH) Universal Credentialing Application®. To get started, please complete or update your CAQH online application. Be sure that you have listed MVP as an authorized plan. For assistance with completion of the CAQH application, please contact the CAQH Help Desk at 1-888-599-1771 or email your question to caqh.updhelp@acsqs.com.
STEP 2
Review the Provider Participation Guide for information on initiating and retaining participation with MVP.
Credentialing Criteria
All practitioners are required to meet the minimum criteria for their specialty. View General Credentialing Criteria (PDF). Email credentialing@mvphealthcare.com with any questions.

Complete and return the following applicable documents:
