  • Medical, Dental and Vision Coverage

    Health Insurance

    High-deductible Health Plan (HDHP)—Two Options

    • Vision and Pharmacy are part of deductible
    • MVP contributes toward a designated Health Savings Account (HSA)

    EPO Plan

    • Includes Vision and Pharmacy coverage
    • Receive reimbursement for activities that promote a healthy lifestyle

    Medical Opt Out

    • MVP co-workers may choose to opt-out of Medical insurance and receive cash back for doing so. You may opt out of Medical insurance and still elect to enroll in Dental, Life and/or Disability insurances.

    Basic coverage is company paid and equal to one (1) times annual salary up to a maximum of $500,000

    Optional Supplemental Employee Life Insurance: Additional 1-5 times annual salary up to a maximum of $900,000

    Optional Spouse/Domestic Partner Life Insurance: 50% of employee’s supplemental life amount up to a maximum of $100,000

    Optional Dependent Life Insurance: $4,000 per child


  • Short & Long Term Disability

    Effective Date: 90 days after date of employment

    Long Term Disability

    • Basic coverage is company paid at 50% base salary
    • Option to purchase an additional 10% of coverage

    Short Term Disability 

    • Basic coverage is company paid at 60% base salary

  • In addition to eight paid holidays each year, MVP employees are entitled to Paid Time Off that includes personal, sick, and vacation time.

    • Salaried employees start with 24 days per year and can earn up to 33 days based on continuous service
