Annual Wellness Visits (AWV) offer more than a yearly check-in. They give Providers a valuable opportunity to close multiple gaps in care, support quality performance, identify risks early, and help Medicare Advantage Members stay on track with recommended screenings, immunizations, and chronic condition support.

MVP encourages Providers to review patient panels and reach out to Members who have not completed an annual wellness visit this year. Proactive outreach can help Members complete recommended care and support performance across key preventive and chronic care measures.

Why Wellness Visits Matter

An AWV creates time to assess a Member’s overall health, review medications, discuss risk factors, and identify preventive services that may be due. It also gives Providers an opportunity to address topics that may not come up during a sick visit or follow-up appointment, including immunizations, cancer screenings, blood pressure control, diabetes care, behavioral health needs, fall risk, and lifestyle goals.

Using the AWV as a care-planning touchpoint can help close gaps in care, strengthen continuity, and support better health outcomes.

Support Quality and Risk Adjustment Goals

Annual Wellness Visits help Providers identify open care opportunities, support accurate documentation, and strengthen care planning. By using the visit to review preventive care, chronic conditions, and risk factors, Providers can help improve quality performance, support appropriate risk adjustment, and promote better health outcomes for Members.

Medicare Advantage Members Can Earn Rewards with an AWV

The AWV helps Medicare Advantage plan Members take a proactive role in their health. During this yearly visit, providers can review health risks, close gaps in care, and create or update a personalized prevention plan. Eligible MVP Medicare Members may also earn a $100 Be Well Rewards reward card after completing their AWV.

How Providers Can Take Action

  • Identify Members who have not completed an annual wellness visit this year
  • Use gaps-in-care reports, patient registries, or other practice tools to prioritize outreach
  • Contact patients directly to encourage scheduling and explain the value of the visit
  • Prepare for the appointment by reviewing open preventive care and chronic condition measures
  • Use the visit to document care needs, update the patient’s care plan, and schedule any needed follow-up services
