MVP HEDIS Operations staff will begin Gaps in Care (GIC) collection for MY26 starting May 18, 2026. From this date forward, be sure to use the latest 2026 GIC report to prepare the records for submission. Using the newest Gaps report throughout the year will save the work of sending records for gaps that have already been closed with claims. It also saves the work of sending documents for patients who are no longer part of the practice or are no longer MVP Members. If you need help obtaining your Gaps reports, contact your Provider Partnership Liaison (PPL) or email mvpgapclosures@mvphealthcare.com.

Providers should carefully follow these guidelines when submitting GIC information:

  • Use one dedicated cover sheet for each submission date, not for each record. The review team needs your practice contact information on every cover sheet, along with your practice TIN, so they can follow up if more documentation is needed
  • Attach each patient record individually instead of sending batch files that aren't separated by Member; this speeds up submission processing time
  • Check that all patient names match those listed on your GIC report. If there's a name difference on any submitted document, include a Member's demographic form showing their previous name after marriage status changes or to confirm suffixes like JR, SR, II, or III. Middle names or initials only matter if they're different between the GIC report and your documentation
  • HIXNY demographic details list all Member aliases for those enrolled. Attach this document if needed
  • Follow NCQA HEDIS specifications as your guide for required documentation and exact timeframes for each measure. Only submit documentation specifically outlined in these specifications
  • Keep a tracking system to prevent submitting duplicates that can't be processed
  • Save time by not sending documentation for Members who are not listed on the GIC report, since it can't be processed
