Help for Managing Multiple Medications and Conditions

If you're an MVP Medicare Advantage plan member with complex health needs or are taking many medications, the Medication Therapy Management Program (MTMP) can help. This program is not considered a benefit and is offered at no cost to MVP Medicare Part D members.  With MTMP, you can talk with an experienced pharmacist.   • How MTMP Pharmacists Can Help 

    MTMP connects you with an experienced pharmacist when it works for you. They can help you:  

    • Understand your condition and your prescriptions 
    • Be sure you’re taking your prescriptions correctly 
    • Check for drug interactions between your medications, including over-the-counter drugs and supplements 
    • Find possible cost savings 
    • Get educational material in the mail 
    • Communicate with your doctor about potential drug interactions or therapeutic duplications 
  • What You Get from MTMP 

    If you participate in MTMP, you’ll receive a:  

    • 10–15-minute phone consultation with a specially trained pharmacist who will provide a full medication review.
    • Written summary of your medication review, including an action plan to discuss with your doctor.
    • Chance to meet with an MVP pharmacist one-on-one, in-person.  
    • Personal Medication List of all your drugs and why you take them. You can also print and complete the Personal Medication List yourself. Keep this list with you for doctor visits or emergencies.
    • Medication review at least every three months. You or your doctor may receive a follow-up letter or a phone call about any new recommendations from the pharmacist. 
  • MTMP Eligibility 

    You qualify for MTMP if you:  

    • Have multiple chronic diseases. For example: osteoporosis, chronic heart failure, dyslipidemia (unhealthy levels of fat in your blood), hypertension (high blood pressure), diabetes, depression, asthma, or chronic obstructive pulmonary disease (COPD). 
    • Take several Part D medications. Our Pharmacy Benefit Manager (PBM) and the MVP Health Care Pharmacy and Therapeutics Committee will determine this. 
    • Are likely to have annual costs for covered Part D drugs that are at least $5,330. 
    • Are an at-risk beneficiary (a beneficiary with an active coverage limitation under a Drug Management Program). 

    If you qualify for MTMP, you’ll receive a phone call and a letter inviting you to participate. The program is voluntary, but if you qualify, we encourage you to participate.

MVP Health Plan, Inc. is an HMO-POS/PPO/HMO D-SNP organization with a Medicare contract and a contract with the New York State Medicaid program. Other physicians/providers are available in the MVP Health Care network. Gia virtual care services are available at no member cost-share for medical plans, including qualified high-deductible health plans (QHDHPs). Exceptions may apply for self-funded plans. In-person visits and referrals are subject to cost-share per plan. Members enrolled in a Medicare Rx plan without additional MVP medical coverage do not have access to MVP virtual care services through Gia.
