Find out about the Healthy Alliance Social Care Network
Healthy Alliance works to address social drivers of health and supports communities facing barriers to improve outcomes. They serve as the Social Care Network (SCN) lead for the Capital Region, Central New York, and North Country under New York Health Equity Reform (NYHER), a component of New York’s 1115 Waiver aimed at improving Medicaid Member’s health statewide.
The Social Care Network collaborates with New York State Medicaid to provide individuals with complimentary access to support services, including assistance with obtaining food, housing and utility services, transportation to social care services, and additional resources.
A primary objective for the SCN is to increase screening volume, ultimately striving to achieve 100% screening of our attributed Medicaid population. If you provide these services or are a contracted screener/navigator, you may be reimbursed for serving eligible Medicaid Members.

Join the Social Care Network!
Become a contracted Provider and help screen, navigate, and deliver services to eligible Medicaid Members! The application is available here and provides comprehensive introductory information regarding the various roles Providers may assume in partnering with the Social Care Network.
