What Is a Formulary?
A Formulary is a list of prescription drugs covered by your pharmacy benefit. We generally cover drugs listed in our Formulary as long as they are medically necessary, the prescription is filled at an MVP network pharmacy, and other plan rules are followed.
The MVP Pharmacy & Therapeutics (P&T) Committee developed and approved our Formulary. The Committee includes practicing doctors, representing a variety of medical specialties, as well as practicing pharmacists and clinical health plan staff members, including medical directors and registered pharmacists. A doctor and pharmacist with training and experience in the care of the elderly and disabled are also included on the Committee.

Our plan covers most Part D vaccines at no cost to you, even if you haven’t paid your deductible.
Important Message About What You Pay for Insulin
You won’t pay more than $35 for a one-month supply of each insulin covered by our plan, no matter what cost-sharing tier it’s on, even if you haven’t paid your deductible.

We cover many commonly used generic drugs at no cost. Tier 1 of the formulary (Preferred Generic Drugs) includes select drugs to treat diabetes, blood pressure, bone health, and high cholesterol. These medications are covered at no cost if you have MVP Medicare Part D coverage:

Drugs to Treat Osteoporosis/Bone HealthAlendronate sodium 35 mg and 70 mg tabs are covered at no cost if you have MVP Medicare Part D coverage.

Other physicians/providers are available in the MVP Health Care network. MVP virtual care services through Gia are available at no cost-share for most members. In-person visits and referrals are subject to cost-share per plan. Members enrolled in a Medicare Rx plan without additional MVP medical coverage do not have access to MVP virtual care services through Gia.
