We all know that collaboration between Providers has a positive impact on the overall health of your patients. When a new MVP Member checks into your practice for an appointment, we encourage you to ask them to complete an MVP Authorization to Disclose Information form to allow you to share important health information with other Providers under their care. This routine should be followed during every subsequent visit as Providers may change, and the form may need to be updated to sustain a more integrative approach to treatment.
