Medicare HOS survey begins in July

The perspectives of your patients are essential in influencing the future of health care. The upcoming Medicare Health Outcomes Survey (HOS) will commence in mid-July and continue until November 1st of this year. This survey is pivotal for collecting important informationregarding the health outcomes of our Medicare patients.

Most care aspects in the HOS survey are covered during the patient's Annual Wellness Visit. The survey includes topics like fall risk reduction, physical activity, mental health, urinary incontinence management, and more.

Please remind patients about the significance of this survey during their visits. Encourage them to complete it if they receive one. Their participation is crucial for enhancing the quality of care and services provided.
