Engage in Open Communication with Patients

It is essential to address health conditions with patients that, while common, may be considered sensitive or uncomfortable to discuss. Even when patients are not currently experiencing these issues, Providers should foster open communication regarding the following topics during wellness visits.

• Urinary Incontinence
• Fall Risks
• Monitoring Physical Activity
• Improving and Maintaining Physical and Mental Health

As you engage your Medicare Members, please mention that if they receive a Medicare Health Outcomes survey in the mail, they should consider completing and returning. Their feedback may result in positive future health outcomes. This survey runs through November 3, 2025.
