November is Diabetes Awareness Month
Published November 2026
With the measurement year quickly coming to a close, MVP urges Providers to review and act on open diabetes care gaps identified in Member-level opportunity reports from their Provider Partnership Liaison (PPL) representative.
Since these reports identify specific MVP Members who need action, they give Provider offices a targeted opportunity to close gaps now, improve quality performance, and support better Member health outcomes before year-end.

This guide summarizes adult HEDIS measure requirements to support accurate reporting and gap closure. For diabetes measures, submit applicable ICD-10 codes E10, E11, or E13 with measure-specific CPT II and lab codes for eye exams, HbA1c/glycemic status, blood pressure readings, and kidney health evaluation, including eGFR and uACR testing.
To help close gaps before the measurement year ends, Providers should take the following actions as soon as possible:
MVP supports Diabetes care gap closure through Member outreach programs and vendor partnerships, including Let’s Get Checked and at-home retinal eye exam services. These resources can help, but timely Provider engagement remains critical to closing gaps, improving quality outcomes, and ensuring Members receive needed care before the year ends.
If you have not received your Member-level opportunity reports, or if your practice needs an updated list, please contact your PPL representative right away.
