Follow these steps to identify eligible patients, document blood pressure readings, and support compliance with the Controlling High Blood Pressure (CBP) measure.

Step 1: Confirm Measure Eligibility

Confirm that the patient has a hypertension diagnosis documented during two outpatient, telehealth, or online assessments on separate dates of service.

Step 2: Obtain and Evaluate the Blood Pressure Reading

At each applicable visit, obtain both systolic and diastolic readings. A reading below 140/90 mm Hg is considered adequately controlled. A reading above 140/90 mm Hg does not meet the measure.

Step 3: Document the Results

Use the required codes identified in the technical specifications to document the patient’s most recent systolic and diastolic blood pressure readings.

Step 4: Follow Up on Elevated Results

  • Review elevated readings with the patient and provide appropriate follow-up care
  • Arrange a repeat blood pressure check when appropriate
  • Document the updated reading and any follow-up actions

How Performance Is Measured

Quality performance is based on the patient’s last blood pressure reading of the year. Because this reading determines whether the measure is met, accurate documentation and timely follow-up are essential.

Take Action

Review eligible patients, document complete blood pressure readings using the required codes, and follow up promptly when results are elevated.
