MVP encourages Primary Care Providers (PCPs), dental Providers, behavioral health Providers, and specialists to share relevant information, as permitted, to support a more complete view of each Member’s health.
When Providers communicate across care settings, they can help identify health risks earlier, reduce duplicative services, support follow-up care, and close gaps in care. This is especially important for Members managing chronic conditions, transitioning between care settings, or receiving care from multiple Providers.
Health needs often overlap. Oral health concerns may be connected to diabetes, pregnancy, cardiovascular conditions, or other chronic needs. Behavioral health needs may affect medication adherence, preventive care, and follow-up after a hospitalization or emergency department visit. Specialty care findings may also help PCPs support the Member’s ongoing care plan.
Sharing the right information at the right time helps ensure important details do not remain isolated within one visit, office, or specialty. A connected care experience supports patient safety, continuity of care, and more informed decision-making.
MVP Care Managers encourage Members who are working with case management to allow their Providers to share relevant health information, when appropriate, to help create a more complete picture of the Member’s overall health and support better care coordination. Providers can also help by reinforcing the value of this information sharing with their patients and responding to care management outreach when additional clinical information is requested. Better care coordination helps reduce the risk of duplicative services while supporting a more seamless Member experience and improving overall Member satisfaction.
