MVP encourages Providers to regularly review and update the Electronic Medical Record (EMR) problem list during patient visits to ensure it accurately reflects each Member’s current and relevant health conditions. An up-to-date problem list supports better care coordination, helps identify gaps in care, improves documentation accuracy, and strengthens quality reporting.

Please use the checklist below during patient visits:

  • Review the problem list at each visit, especially annual wellness, chronic condition, and follow-up appointments
  • Confirm that active diagnoses are current, clinically supported, and reflected in the care plan
  • Resolve or remove outdated, duplicate, or inactive conditions when appropriate
  • Add newly identified conditions based on the Provider’s clinical assessment and documentation
  • Use clear, specific diagnosis descriptions instead of vague or general terms when possible
  • Ensure chronic conditions, medications, referrals, screenings, and follow-up plans align with the active problem list
  • Update the problem list when a condition changes, improves, resolves, or requires a new course of treatment
  • Reinforce team workflows so clinical and administrative staff understand when problem list updates may be needed
  • Use the updated problem list to support care coordination, quality reporting, risk adjustment, and proactive Member outreach

Regularly reviewing and updating the EMR problem list helps ensure each Member’s current health status is accurately documented. A complete and current problem list supports coordinated care, appropriate follow-up, quality improvement, and better identification of gaps in care.
