Complete Your NCQA-Required Training

NCQA requires health plans to provide training to Participating Providers on topics of health equity, including cultural competence, bias, diversity, and inclusion.

To best serve the cultural and ethnicity needs of our members, we are asking all Providers to take a training and attest to its completion. A list of New York State training options are available here.

Please ensure all providers in your group have completed a training, then submit the form below. Upon submitting this form, the authorized representative will have attested that all providers within the practice have completed an annual cultural competency training, and that all those who join in 2023 will complete the training.

Please Note: Vermont Providers not serving NYS Managed Medicaid Members may complete an alternative but equivalent training.

If applicable, you may attest at the NPI level.
